At NeuroTribes Australia, our Specialist Support Coordinator helps participants navigate complex systems, manage crises, reduce the risk of service breakdowns, and coordinate intensive supports across multiple providers and agencies such as healthcare, housing, mental health, child protection, or justice services.
The role focuses on stabilizing the participant’s situation, improving safety and wellbeing, and building a more sustainable and effective support network.
Developing strategies and action plans to help manage emergencies, high-risk situations, or periods of instability to ensure participant safety and continuity of supports.
Coordinating and maintaining communication between multiple providers involved in the participant’s care, especially when challenges, conflicts, or service issues arise.
Identifying potential issues that may disrupt supports and putting measures in place to maintain stable and consistent services for the participant.
Assisting participants in understanding and accessing complex government, healthcare, legal, or housing systems while coordinating necessary supports and services
Working closely with various organizations, providers, and government services to ensure all supports are aligned and effectively meeting the participant’s complex needs.
• Specialist Support Coordinators work with participants who have highly complex circumstances, helping them navigate multiple challenges such as housing instability, mental health concerns, justice involvement, or complex disabilities.
• Identify potential risks early, develop strategies to prevent crises, and coordinate urgent supports when unexpected situations arise.
• Bringing together healthcare providers, hospitals, mental health services, housing providers, government agencies, and community organizations to ensure everyone is working toward the participant's goals.
• Participants receive tailored guidance to build confidence, develop problem-solving skills, and gradually increase their ability to manage their own supports where possible.
• Specialist Support Coordinators identify and connect participants with services that can effectively meet their unique and complex needs, even when suitable supports are difficult to find.
• They advocate for participants when navigating service barriers, resolving provider issues, or working through significant life transitions to ensure their needs and preferences remain central.
We collaborate with various funding providers, including the National Disability Insurance Scheme (NDIS), to facilitate access to our services. Our team can assist you in navigating funding options to. ensure you receive the support you are entitled to.
